If your Club Sport team is traveling more than 25 miles from campus, you must give the Office of Student Life the following information:
- A Pre-Event Planning Form
- A Student Travel Request Form
- A signed waiver for all participants, including an emergency contact and any medical insurance information available. The Assumption of Risk Form must be printed and page 1 should be completed by the Student Organization officers and then copies should be printed for participants to complete page 2. All forms turned in prior to travel must include both pages.
If your organization is renting a 15 passenger van, the driver must take a driver safety course before using the vehicle. Please see the Office of Student Life to find out how to take this online course.
A copy of all information above should be taken on the trip with the organization. One copy will remain with Student Life and another copy will be made and given to University Police.
