Club Renewal Form

Submitted each spring semester after the club is organized, so that Rec Sports has a record of all Officers.

Pre Event Planning Form

Used to help outline and detail club events and activities, so that they are run smoothly and efficiently. Also how Rec Sports approves events.

Student Travel Request

Used to plan travel for Club Sports, and provides Rec Sports with important info in case of emergencies.

Post Event Form

Used to reflect on  Club Sports events, and helps make future events more effective.

Assumption of Risk and Waiver Form

Generic fill-in-the-blank waiver that can be curtailed to all Club Sports events.

Event Publication Request Form

Used to ask Rec Sports to publish information about Club Sports events.
